Directly from you (in person, by phone), from your guardian or responsible person, online and from other health professionals and others with your consent. From your health fund, Medicare, or the Department of Veterans’ Affairs (as necessary).

Provide safe and effective allied diabetes health services.
Communicate with you and coordinate care with other providers.
Manage billing, administration, and compliance obligations.
Meet legal and regulatory requirements.
We will only disclose personal information in accordance with the Act. This means that personal information may be disclosed:

However, Diabetes Education to Assist may occasionally deny such a request in accordance with the exemptions contained in the Act.
